- You have the right to be treated with respect and dignity.
- You have the right to fair treatment regardless of your race, religion, gender, ethnicity, sexuality, age, ability, etc.
- You have the right to have your treatment and records kept private and confidential. Only as required by law can your records be released without your permission.
- You have the right to a simple explanation of your diagnosis and treatment plan.
- You have the right to participate in your treatment plan. Your plan of care is a collaborative process between us to best help you succeed in your goals.
- You have the right to know about laws and policies from the State and professional associations that govern the profession and practice of psychology.
Statement of Client Responsibilities
- You have the responsibility to treat your health care providers with respect and dignity.
- You have the responsibility to give your health care providers all the information they need so they can deliver the best possible care.
- You have the responsibility to keep your appointments. You must call to cancel within 24 hours if you cannot make your scheduled appointment.
- You have the responsibility to follow your treatment plan.
- You have the responsibility to give your health care providers feedback of what works and what does not work in your treatment plan so your care may be adjusted accordingly.
- You have the responsibility to actively participate in your own growth and healing.
